You need to write each diagnosis and the plan. \\

- Daily assessment of mood. \\
- Recommend outpatient followup with therapy\\

- Psychoeducation about PTSD and substance use issues \\
- Recommend outpatient followup with therapy using exposure based principles when patient no longer is in major depressive episode. \\

By doing it this way you will learn what the treatments are for each disorder.
