I cannot sign notes that say:

1.Psychiatric
- Prozac 20mg qd.

Instead write
# MDD recurrent moderate
- Prozac 20mg daily

Please don't simply replace Psychiatric with a list of diagnoses. Have a plan for each diagnoses. For example:
Major Depressive, Recurrent, severe without psychotic features
Post Traumatic Stress Disorder
Borderline Personality Disorder
- Prozac 20mg - Standard PRNs
You need to write each diagnosis and the plan.

Major Depressive, Recurrent, severe without psychotic features
- Prozac 20mg qd
- Daily assessment of mood.
- Recommend outpatient followup with therapy

Post Traumatic Stress Disorder
- Prozac 20mg qd
- Psychoeducation about PTSD and substance use issues
- Recommend outpatient followup with therapy using exposure based principles when patient no longer is in major depressive episode.

By doing it this way you will learn what the treatments are for each disorder.
